Gracedale Had Two Serious Deficiencies in September and Failed to Notify NorCo Council

The long-awaited first-ever meeting of the Gracedale Oversight Committee was finally conducted yesterday afternoon. Though there are only four members, all of  NorCo Council, with the exception of Jeff Warren, were present. This heavy attendance is what'll happen when an Executive proposes a 43.5% tax hike and wants $12 million to go to Gracedale. 

Chair Kelly Keegan, who voted against establishing a Gracedale Oversight Committee, explained yesterday why she waited so long to call a meeting. She said she wanted to give Tom Carson, the newly appointed Director of the newly created Continuum of Care department, some time to familiarize himself with his new role. She said she'd take responsibility for the delay, and I can accept her reasoning after watching him yesterday. I'd say he even needs more time.

It was a very lengthy meeting and I will provide more detail in a separate post. But for now, I want to focus on two September inspections that resulted in G-level deficiencies — a classification described as involving actual harm to a resident. We'll learn more about them when the state Department of Health (DOH) posts its surveys. There was also a third, less serious, deficiency.

At a meeting in early February, Council member Dave Holland requested that County Council be immediately notified whenever DOH visits Gracedale because there is a 41-day lag between when a visit occurs and when it is posted online. He was assured that Council would be notified, but that never happened. 

Holland: "So you're telling me you had a survey on September 10 and there was a G?

Administrator Michelle Morton: "Correct."

Holland: "That's the first time this County Council is hearing that. Does anyone else have that information?"

Morton: "I believe that was sent in." 

Holland: "I'm telling you No. Whose responsibility is it to notify this Council when we have surveys because that did not happen? Anybody in the room? Anybody? Whose responsibility is it to do that?"

At this point, Executive Tara Zrinski said that function would have been up to the Human Services Director but now belongs to Carson. She apologized that she failed to make that duty clear to him.

This communication failure is significant because Gracedale remains on a Provisional II license. Pennsylvania records identify Gracedale as operating under that provisional status, following deficiencies that resulted in the downgrade.

As miffed as he was about the failure to notify Council of two serious deficiencies, Holland said the more serious issue was the new G-level deficiencies and what it could mean for Gracedale's provisional license. He had been encouraged that Gracedale might be moving toward getting out of provisional status, but that new G-level deficiencies could make that more difficult.

Zrinski acknowledged that losing the license was theoretically possible but argued that the latest incident was different from previous problems and that the appropriate response was to correct the problems rather than assume the facility would fail. "We are not going to lose this license," she flatly insisted. She said remained optimistic.

Council member Theresa Fadem was less optimistic. "I support optimism, but what's going to happen if we lose this license? Why are we being asked to fund another $12 million? I don't understand what is going on here. We're throwing money at a problem and we could just lose Gracedale. What is happening"

Zrinski rejected the notion that the county was simply "throwing $12 million" at Gracedale. She said the $12 million represents a budgeted county contribution based on what the county wants Gracedale to become, rather than what the facility looks like today.

She downplayed the G-level deficiencies saying "they are not as horrific as you think. We are not going to lose this license." She argued Gracedale only gets more attention because of a critical Grand Jury report. She also blamed agency nurses, claiming, "We need our eyes on people. That's how things like this happen."

Fadem remained unpersuaded. "How do we expect to attract more people to Gracedale when we keep doing the same thing over and over again and expect different results? Isn't that what somebody called the definition of insanity?"

Zrinski: "I do understand the definition of insanity." She then went on to say that other nursing homes have problems, too, referring to an incident at Moravian Hall Square. But Fadem noted that Gracedale, unlike Moravian Hall Square, is a publicly run facility. Zrinski then said she is putting out fires daily.

Council member Keegan, who is a nurse, told Zrinski that she would be sure to keep her eyes on patients with histories of problems. She said she wants to hear that every single employee is staring at the residents to ensure they don't fall, they don't choke or have other issues that can be prevented. Zrinski sarcastically suggested bubble wrap, but no one laughed "We want to hear that employees are taking extra care because this is serious," said Keegan.

Zrinski then got emotional. "Things can happen in a second. And when they do, you think, 'I should have been watching." And you blame yourself. You can't think that all of us sitting here today haven't thought, 'Damn it. We failed again.' because we do. That goes through our heads. And we're trying so hard. This is not easy."

She then returned to her seat.

This emotional exchange indicates that Gracedale's problems are beginning to take a toll on Zrinski, who does seem intent on keeping a poor administrator and unwilling to look into other publicly run nursing homes. 

The next Gracedale Oversight Committee will be December 2 at 4 pm.

  1. Mortin needs to go, carson is way over his head and has no idea of anything nursing home related, not the best talker ether; kept looking for help when he should be running the show. And tip my hat to kelly she did an excellent job leading the meeting. When do we raise the white flag and say sorry we may not be able to save this sinking ship?

  3. Did not feel a sense of urgency and Heard no discussion about bringing a “professional nursing home management team” for oversight of the home.Did TZ contact/visit successful nursing homes to ask for advice?
    This committee, the new director, Kraft, Keegan, and TZ, regurgitated problems they knew existed for years under their governance. Nothing new to see here.
    By refusing to tap into Holland’s past expertise and success in managing Gracedale, under the Premier Nursing Home program,
    this inexperienced committee will
    continue to verbally tread water and spin narrative while the clock runs out on patients and employees.

    1. I was assistant Maintenance Supervisor at Gracedale when Holland was there. He knew how to run Gracdale and did a great job.! Ask him if he wants the job again!

  4. Of 67 counties in the Commonwealth of Pennsylvania, just 11 own or operate nursing homes. It's no longer a core function of county government! This is a 1.25 mill disaster for the taxpayers.

    God help us. Can we please elect a leader who doesn't crumble when she's weeping through a "girl day?" She is immature, unprepared, overwhelmed and completely disgraceful. And Keegan is going to keep an eye on things? Really? I wouldn't trust her to open a Band-Aid. She's demonstrated a complete lack of concern for a murder victim's widow and her children, and doubled down again and again on her viciousness. She's vile, and shouldn't be anywhere near patient care. Ask those who know her well. Ironically, this group of Gracedale protectors will be the ones who ultimately close the place. Stoffa was right.

    1. I allowed this comment but the “girl day” remark is sexist. Zrinski is allowed to be emotional and show passion. She cares but this is no evidence that she is taking the right approach. She plays the blame game a lot instead of recognizing her real problem is her administrator.

      As far as Keegan is concerned, she ran a good meeting and made some valid points including the need for employees or agency nurses to be more vigilant

    2. A good portion is salary and benefit increases. The proposed budget has a step (4%) plus 1% for nonunion career service. On top of that 67 positions are being upgraded. On the benefit side, unlike last year the pension is being funded properly. The vision and prescription coverage is up 37% or $2.3 million!

      On a previous thread, someone mentioned OPEB. Stoffa ended that in 2010, and it is adequately funded for now. Council could save $2.9 million by cutting those contributions from the budget.

  14. Zrinski is exactly what I suspected she would be. She’s a nice lady. She has no executive experience. No leadership experience. She’s in way over her head. She’s emotional and can’t handle pressure. Her star hire Carson can’t even answer basic questions about the facility he supposedly runs. This was all easily predicted. As far as the 43%, all my neighbors were talking about it , and they generally don’t follow county politics. Not good for her. The best thing she has going for her is the Republican Party in this county is just as incompetent.

    don’t you think? So it would appear they ( Tara, Morton and Carson) knew about the first G tag citation since September 10th and said NOTHING!! Oh right, forgot, Tom didn’t know it was his job to report anything, right? So now he does. This is a disgraceful attempt to cover up what appears to be resident abuse (two Gs and a D won’t help a Provisional II license) by neglect at the very least. I guess we have to wait until it’s posted to the state facility locator site. Tara rationalized this by saying “Other nursing homes have problems too, look at Moravian Hall Square” and somehow blamed the grand jury report for the reason Gracedale can’t attract good people. Three people technically “running Gracedale Nursing Home” in its administration with TR degrees. Sure, that’ll work. Tears were certainly dramatic from Tara but meant nothing, A supposed defiant Kelly Keegan meant nothing either. That interaction was most likely rehearsed. Give me more Theresa and David! Where was Jeff Warren? He’s such a supporter of Tara. Oh right! He has a campaign to run. Gracedale, its residents and hard working loyal county staff (if there’s any left) are in trouble unless this is turned around quickly.

    1. Managers continue to consult with incoming managers. So wandalowski knew to tell Carson to tell council. When is she called out? How would he know if he wasnt told? Every manager is required to write a job description in detail. Did Sue write one for Carson with this detail? Would be nice if exec asked her this. She has assistants to do the job previous human services directors did plus gracdale. Why in the past 8 years has it all, morale included gone to shit? County exec made a mistake keeping current human services director in when any other executive would replace with better choice. All around place needs an overhaul on management. Council should ask to read these internal job descriptions Sue requires for succession planning. Very interesting reads on the share drive.

  16. All very predictable. A county executive with no experience, no leadership skills, and an ideology that believes government is the answer to everything. A county council that’s just as incompetent. A political hire who has had plenty of time to get settled and can’t even answer basic questions about the facility he supposedly runs. A 43% tax increase that is way more than needed. It’s like watching a slow motion train wreck. As far as the so called sexist comment, leaders must control their emotions to lead. It’s just more evidence that she’s in way over her head.

  17. The presentation was very poor. Organized, even incoherent. There was lots of talk but little clarity in what the administration plans.
    If I had given a presentation like that at work it would have been cut off after 3 minutes and I might have gotten fired.

  19. Unfortunately, you are dealing with a combination of Gracedale being completely broken and the doh morphing into a real monster like we have never seen. G level deficiencies are relatively common now, but with the past history of Jeopardies at the facility, you have no room to breathe. You’re in danger of losing the operations to a management company assigned by the state of Pennsylvania. You almost forget that Lehigh, Berks, Bucks run nursing homes as they are rarely in the news. Bernie, in the recent past you listed an extensive list of county homes who do well, including them. The obvious step for TZ is to talk to those counties. I have not heard it reported that she has talked to any of them, unless it happened on the down low. Each one would have populations similar to the population at Gracedale and be resources to help course correct. Better learn from them before it is too late. Pro tip - never ever talk about other specific Nursing Homes having issues. It is highly unprofessional for TZ to talk about Moravian hall square, particularly when there’s no negative survey listed on the site this year. Another pro tip, for the money you spent on therapeutic recreation professionals you could’ve received hundreds of consulting hours per year from a professional Nursing Home company. Making up two bs positions that do not exist anywhere in ltc industry is such a “government” type of solution.

  20. The presenters lacked facts. The most interesting item was that the Gracedale population is 450.
    It was closer to 500 the last couple years and in theory can hold 688 people.

  21. Great information but good for DOH for finally cracking down on resident harm citations. Tara continues with her own agenda of expecting individuals with TR degrees and in the latest addition, no nursing home experience, to solve the problems of this provisional II license with an abuse warning with 2G and a D in September. How can you operate this size facility when no one has a clue?
