Gracedale Has Reduced Reliance on Agency Nurses to 57%, But Is Still Operating at a Loss

Though Northampton County's Gracedale is currently its biggest headache, nearly all of yesterday's meeting of Council's Human Services Committee was devoted to other departments. Administrator Michelle Mortin had only 15 minutes to update Council. She started by passing out a chart covering different data collected, but it was only presented to Council. As a result, I can only rely on what she stated in her description of her handout.

Gracedale staff v. Agency nurses. - According to Morton, agency hours are 57% to 43% by Gracedale staff. She stated this is a significant improvement over the 75% reliance on agency staffing one year ago. As a result, Gracedale has spent less on agency nurses. 

Hours per patient per day (PPD). - The state minimum is 3.2 hours per patient per day. Morton stated that Gracedale has consistently been above this minimum threshold since it was cited for being below this minimum on November 18, 2025 and again on January 8, 2026. 

Census remains below 520. - Morton acknowledged that its census remains below 520, which is the minimum census required to break even. She explained this is because the nursing home has been more selective with its admissions. 

Non-nursing Overtime Reduced. - Early last year, non-nursing staff were averaging 240 hours per month in OT. That has been reduced to 40 hours per month. 

Gracedale is in the red. - In January and February, Gracedale had operating losses. But she said that there is significant unpaid revenue because if takes about 300 days to get someone approved for Medicaid. Once that revenue is realized, she expects Gracedale to be in the black. 

Gracedale's residents. - She noted that 85% of the residents have psychiatric diagnoses, compared to 60% on the state and national level. She added that 17.5% are under 65.  This population has a negative impact on quality measures because of the medications that most be used. 

Gracedale's One-Star Rating. - is due to the exorcist nurse and staffing shortages. As a result of the nurse who assaulted a resident, Gracedale will be stuck at one star for a year

Department of Health Visits. - There have been seven visits from the state Department of Health so far in 2026. Aside from the staffing deficiency found on January 8, there have been no additional deficiencies.

Initiatives. - The home is currently conducting dementia training through a state grant to improve empathy and care quality. It has begun an infection detection project to reduce transfers to a hospital and improve detection in dementia residents. It has an ongoing falls reduction strategy

  1. Council is not supposed to talk about Gracedale publicly. So, I have a few questions if council did not.
    1. How did they achieve the agency nurse decrease? Was it by hiring more nurses or playing with numbers?

    2. Was the 15-minute limit ordered by council or the administration?

    3. We know that because of the former executives political run, some on council as well as Zrinski want to downplay any problems. (no 4th quarter financials) Why such short sift given to Gracedale. Did windy Warren order it?

    1. County Council does and is required to discuss Gracedale publicly. What I am saying is that the spreadsheet provided to Council is not posted anywhere that enables the public to access it as well. I can get it with a simple RTK and will start doing so.

  5. Gracedale is fine. It’ll need a 7m bridge loan every year from the County to balance books, but otherwise they’ll be able to carry out the mission of caring for the County’s poorest, sickest, Seniors.

    1. @ 7:26.. what an ignorant comment. “Poorest and sickest”? Unfortunately not everyone is as wealthy as you and your family. However, you should know that my grandmother (who was wealthy for many years) ended up at Gracedale due to dementia. The “for profit” homes drained her dry and kicked her there. While at gracedale she actually had far more better days then she did at the for profit home that just laid her in bed and collected their monthly fees. You don’t know why people end up there and too be honest with this world today (ie. R’s) running the show we may need more of these places!

    3. @453.. Don’t stress about the stiffs leaving these silly comments. They either think they will never get old and need such a facility or they are hard core red hatters who feel a good government action is “invasion” of their rights or some crap. Oh maybe they think if the county dumps Gracedale they will save a whopping 40 bucks a year in taxes because county would lower them, which they shouldn’t even if, hopefully they don’t, but even if they sold Gracedale.

  8. Eighty five percent have a psychiatric diagnosis. Gracedale is the new state hospital. Its Quality Measures will never improve and its events will continue to occur at that rate. Her report would be very interesting to view. Go for it.

    2. Every time one of these patients end up in the hospital emergency room for a urinary tract infection or dementia related behaviors, they are coming out with a psych diagnosis. Unspecified Psychosis or Neurocog.
      PERIOD.

    1. In accordance with modern best practices, Gracedale exorcisms average less than 45 minutes now.

  10. I have a few questions: 1) "Consistently above this minimum threshold" - were there any instances where Gracedale was below? If so, how many and what is the potential impact of this failure? 2) You are reporting financials on a cash basis, not accrual. Why? If the county finance group is not able to produce accrual statements, what is the Administration doing to build these skills so that this critical information can be reported in a timely fashion? 3) You report that Gracedale is in the red, how is Gracedale doing vs. the budget that was submitted in support of the County contributing $7 million to cover the deficit?

    I could go on for another hour asking questions - my concern is that the Council is not. Either they are not versed in financial topics (scary), they're ignoring the problem again (also scary), or they are complicit in a poorly run operation (really scary).

    1. You really need to go to Council and ask these questions. Make sure the Executive, Controller and Gracedale Administrator are there, so you can get specific answers. You won’t though because you’re a fraud.

  12. The math is simple as is the other data. As long as Gracedale continues to admit significant numbers of MA pending residents, it will continue to wait for payment for 300 days (unless PA improves that process). As long as Gracedale continues to admit significant numbers of residents (to maybe increase census) with psychiatric diagnoses, they will continue to have an above average percentage of these residents. There now appears to be several key positions recently posted as vacant at Gracedale. Perhaps they were planned. Interesting that those three commissioners toured Gracedale. I’m certain that was known in advance and Gracedale put on its best face for them. The only one of those three who could identify anything, however, would be Holland. A lot of the math appears to be fuzzy. Where was the report of the “mock survey “? Oh guess there was only fifteen minutes allotted for the Gracedale presentation so that couldn’t be discussed. I don’t think many truly want Gracedale to fail but hopefully council can see through that smoke. It was visible on the You Tube video.

  13. Gracedale should not be in financial trouble. There was funding in place, specifically , for Gracedale funded by an Intergovernmental transfer program done with other county owned contributing homes that created $2 million dollars per year. It protected the Northampton County taxpayer from having to be burdened with raising taxes or using general funds. NO one has answered a question as to where this program and funding has gone? It should have been tagged for Gracedale if it still exists.

  15. You cant just cut agency maintain a census and still meet hours. Overtime I n nursing had to go up. Will be important to see report. Carefully thought out presentation to hide the facts im.afraid.

  16. Maybe someone else besides Michelle Morton should ”gather” the data. If the numbers presented are not honest and accurate why bother? The county needs to hire an independent auditor. How did agency usage decrease so fast?

  17. Meanwhile Council gave 30K to some dude to once again conduct a "study" that will tell them Norfolk Southern controls the rail lines and it's faster to take a bus to NY.

    1. The Igt remains as far as I know. I suspect with less county homes less to invest thus less back to homes but also less homes to take back profit.....not sure in dollars how that works now.
